The post holder would be part of a pro-active multidisciplinary team responsible for ensuring that the practice provides optimum care relevant to the practice population, either in practice or the home environment.
The post holder will work collaboratively with the whole general practice team to meet the needs of patients by using motivational interviewing techniques in supporting patients in taking responsibility for their own long term conditions
